Charles (Chuck) R. Mitchell, DDS., born in Chicago on March 21, 1937; died of natural causes February 10, 2010 in Scottsdale.

He will be buried with full military honors at the National Memorial Cemetery in Phoenix following an 11:00 a.m. Mass at Our Lady of Joy Catholic Church, February 19, 2010.

After graduating from St. Rita High School in Chicago, Chuck attended Illinois Benedictine in Lisle, Illinois, where he met Arlene Thollander of Naperville. They were married while Chuck was at Marquette University Dental School, where he received his DDS in 1961.

Dr. Mitchell served as Captain in the Air Force, seeing duty during the Cuban Missile Crisis. While practicing dentistry in Downers Grove, Illinois, he co-founded Grove Dental Associates and founded Associated Health Services, which led to the establishment of managed dental care nationally; and was Treasurer/ Fellow for the Academy of General Dentistry.

Dr. Mitchell was Chief of Dentistry at Hinsdale Hospital and helped found Good Samaritan Hospital in Downers Grove. He led many Catholic Youth Groups and was a member of the Board of Directors for various banks.

He was predeceased by his parents, Rita and Charles Mitchell, of Lake Forest, Illinois. He is survived by his wife of 51 years, Arlene Mitchell; son, Stephan, and wife, Dawn, son, Edward, and wife, Lynnette; daughter, Cathy Keeler and husband, Doug; grandchildren, Kelsey, Ryan, Max and Sam Mitchell and Carly and Ian Keeler; siblings, Mary Lou Fort, Jack, Bob and Mike Mitchell and their spouses.
