His first wife was Martha Rosa Taylor and they married on 17 September 1887 at Tate, Pickens County, Georgia. She preceded him in death on 2 April 1899 at Sugar Valley, Georgia. After her death, Dr. Cagle moved to Texas.

Two children were born to this union:

Dr. W.S. Cagle married Miss Susan Victoria Keeter on 24 February 1901 at her parent's home in Eliasville, Texas. She preceded him in death.

Dr. W.S. Cagle, 84, pioneer Throckmorton County physician practiced medicine in Young and Throckmorton counties for 50 years. In serious ill health for about a month, Dr. Cagle retired in 1943. He was raised and received his medical training in Georgia. The physician came to Young County, Texas in 1876 settling first at Proffitt. He practiced medicine there and then in the Woodson community in Throckmorton County. In 1919, he moved to Elbert, Texas.

Services were held at the Elbert Baptist Church with Rev. Hannah officiating. He was buried beside his wife.

Survived by his two sons - Joe of Woodson and Perry of Littlefield; four daughters - Mrs. Maxine Pendergrass of Graham, Mrs. Preston Bruton of Elbert, Mrs. Emily Grady of Georgia, and Miss Alma Cagle of Miami, Florida.
