Son of Joshua Jack and Eliza O'Neal, he was born in Licking, Blackford County Indiana, of "Irish stock on both sides" as he stated in his will. He married at age 27 to Amanda Dardin Campbell, who died October 22, 1912 near Muscotah, Atchison Co, KS, and she is buried in Muscotah Cemetery. They had seven children, only two reaching adulthood, Gerald Elsworth Jack and Grace Velma Jack Koehler. They also had an adopted daughter Mary Dorine Fury Nation who was left on their doorstep at about age two weeks. They raised her as their own, she lived to adulthood, married and lived most of her life in Iowa.
