Polixene was the daughter of Ernst, Margrave of Hesse-Rheinfels. On 20 August 1724, she became the first wife of King Carlo Emmanuel III of Sardinia.
They were the parents of 6 children-King Vittorio Emmanuel III, Maria-Leonora, Maria-Luisa, Maria-Felicita, Emmanuel Filiberto, and Carlo Francesco.
