The State of Alabama,

County of Chambers

Personally appeared before me, Lloyd Robertson, Judge of Probate in and for said County, Mrs. Mary Willoughby, who, being duly sworn, upon oath, says that her husband, J. T. Willoughby, was a private in company "D" of 14th Regiment of Ala. Volunteers; that he came to his death from wounds received or disease contracted in the service of the Confederate States, and that his death occurred during the late war or within five years thereafter; that she has not since remarried; that she was a resident of Alabama on the 13th day of February, 1891, and is a resident at the date of this application; and that her taxable property does not exceed four hundred dollars in value.

Sworn to and subscribed before me this 30 day of May 1893.
