Corporal, from 2 Canadian Mechanized Brigade Group Headquarters and Signals Squadron based in Petawawa, Ontario. Aged 25, he was from Waasis, New Brunswick. He was deployed with the Canadian contingent of NATO forces in Afghanistan.

He was killed along with WO Dennis Brown and Cpl Dany Fortin after their armoured vehicle hit an improvised explosive device in the Arghandab District approximately 10 kilometres northwest of Kandahar City. They were returning to base after defusing another roadside bomb. Two other soldiers injured in the incident were evacuated by helicopter for medical treatment.

Recipient of Sacrifice Medal (posthumous)

Cpl O'Quinn was born in Happy Valley-Goose Bay, NL and spent his childhood there before the family moved to New Brunswick. He attended high school in Oromocto and worked out at the gym on the military base in Gagetown, where his mother is employed. Chad joined the military in June 2002 and previously served in Afghanistan in 2005. He was described as a consummate professional and a charismatic leader with a great future.

He highly valued his faith, his health and loved ones. Throughout his two tours, he telephoned, emailed and messaged his family each and every day. His death happened just days after his grandfather, Alfred Decker, passed away in Canada.

Chad is survived by his fiancรฉe Naomi Holder, parents Rhonda and Kenneth, brother Adam (Myriah), niece Alexis, grandparents Alex and Rosaline O'Quinn and Jean Decker, and extended family. Also left behind is Onyx, his beloved pitbull.

Funeral service held on Saturday, March 14, 2009 at St. Luke's Chapel, CFB Gagetown, NB. Interment in Ottawa on March 25, 2009.

The deaths bring to 111 the number of Canadian troops killed since the country's forces arrived in Afghanistan in 2002. One diplomat and two aid workers have also died.
