On 24 Dec 1889 Ellsworth married Alice Flora Lindsey in Pickaway County, Ohio. Two daughters were born to this marriage: Grace and Sylvia Marie. (Grace married Albert C. Leist and Sylvia Marie married Milton P. Manson.) In May, 1898 Ellsworth and Alice Flora were divorced.

Enlistment: May 27, 1898, joined the 19th Regiment of the Infantry - Company M. Age at enlistment was 29; occupation Clerk; eyes brown; hair black; complexion fair; height 5'6".
He was at a camp just outside Tampa, Florida, for basic training and/or combat training. He left there for Puerto Rico to participate in the Spanish-American War. He endured a rugged march cross country and suffered foot and respiratory problems. Filed for Pension 19 Jun 1920, age 51, b. 19 Sep 1868, discharged 09 Mar 1899 at Ponce, Puerto Rico, then lived at Circleville, Ohio and National Home, Wisconsin. Partially disabled. Old injury to right
foot resulting in deformity while in service. He passed away at the soldier's home at Pheobus, VA.
