Stephanie Marie Foreman, 50, of Antwerp, Ohio passed away Thursday afternoon, January 6, 2022 at Parkview Regional Medical Center in Fort Wayne, Indiana.
Stephanie was born on March 4, 1971 at Adams County Hospital in Decatur, Ind. to Sandra Clay and the late Myles Foreman.
Stephanie was preceded in death by her father, Myles Foreman as well as her grandparents, Donald and Lucille Foreman and Kenneth and Elva (Sally) Ross.
There will be no services for Stephanie.
