Funeral services for Donald E. Cook, 36, of 3362 Sylvia St., Jacksonville, Fla., were held May 22 at Assumption Catholic Church in Jacksonville. Mr. Cook, husband of the former Ann Forsterling of Sheboygan, died Tuesday in Jacksonville after being stricken with an apparent heart attack. He was employed by the S.B.S.A. Insurance Co. in the Jacksonville area. He was a U.S. Navy veteran. Survivors include his widow; a daughter, Margret Ann, age 2, at home; and his parents and two sisters of Lawrenceburg, Ky.
