PBY-5A Catalina, Bureau № 48386, departed from Cold Bay, Alaska, for Whidbey Island, Washington, with an intended stop-off at Kodiak Air Station, Alaska.

While attempting to land at Kodiak, the airplane struck the base of Old Woman Mountain, adjacent to the air station. Eight Navy personnel aboard were killed in the crash and seven survived and were immediately rescued by the base's personnel.
