He is survived by a sister, Kathryn Smialek and her husband, Stanley, of Lake City; two brothers, Earl E. Cook Jr. and his wife, Dawn, and David W. Cook, all of Girard; his maternal grandmother, Anna M. Walsh of Erie; good friend Ronald E. Albaugh; and several aunts, uncles, nieces and nephews.
