Carolyn is survived by her sons: Zach A. Cook and Michael A. Cook, both of Findlay; her brother, David Brown; sisters: Nicole Brown, Stacy Trimble and Kelly Rizor.

A visitation will be held 10:00-11:00 a.m., Friday, July 2, 2021 at COLDREN-CRATES FUNERAL HOME, Findlay.
