Jordan Brooks of East Patchogue sadly passed away on May 20th, 2021. She was born on June 12th, 2003 in Stony Brook to parents Carolyn Hernandez and David Brooks. She attended and graduated from Patchogue Medford School District. Jordan had a passion for art, being outdoors and enjoyed crocheting and thrifting. She will be forever remembered by her loving parents, Carolyn Hernandez and David Brooks, brother Jayson Brooks and grandparents Paul and Eileen Dunca and Richard and Joan Brooks. Visitation for Jordan will take place on Monday, May 24th from 7 to 9 pm at Robertaccio Funeral Home, Inc. in Patchogue. A Funeral Mass will be celebrated 11 am on Tuesday, May 25th at Our Lady of the Snow RC Church in Blue Point. Interment will follow at Mt. Pleasant Cemetery in Center Moriches.
