Cynthia Susan Hazlett Mathias Wells, 65, of 420 N. Webster, died at 7:48 p.m. January 17, 2015, at Select Specialty Hospital in Davenport. She was born November 21, 1949, in Ottumwa to Charles and Helen Yacko Hazlett. She married Ray Mathias. They later divorced. She married Phillip George Wells. They later separated.

A graduate of Ottumwa High School, she had worked at John Morrell & Co., at Chamberlin’s in Albia, for the Ottumwa Community Schools, as a cook at several nursing homes in Ottumwa and at Arby's. She enjoyed watching wrestling with friends, bingo, bowling, shuffleboard, dancing and music.

Surviving are a son, Bill Mathias (Windy Aquino); a daughter, Jennifer Shively (Jerry Bantz); a granddaughter, Sierra; several aunts and uncles including a special aunt, Verna Yacko of Albia; a brother-in-law, Richard Hatfield; a niece Joni Hatfield; two nephews, Jim and John Hatfield; and many friends.

She was preceded in death by her parents and an infant brother, Jimmy Hazlett; two sisters, Linda Hazlett and Connie Hatfield.

Her body has been cremated. Visitation with the family present will be from 6 to 8 p.m. Wednesday at Reece Funeral Home. Memorials may be made to the family.
