Derek James Travis of Swan Lake, NY died Thursday, January 14, 2021 at home in Swan Lake, NY with his father, Glenn Travis where they resided with his mother, Tammy. He was 18.
He was born August 21, 2002 in Port Jervis, NY the son of the late Glenn Travis and Tammy Mann Travis.
Derek was a BOCES student at the Monticello Central School.

A family statement read: “Glenn and Derek were inseparable. They were two sides of the same coin. They were always together, and will always be together. They had a one of a kind relationship. They loved to play video games together and tinker with their cars. They were never apart. Where one was, you would find the other. Every breath they gave was for our family and the love we shared. They will forever be loved and missed. There are no words to explain our loss. Until we meet again. We love you both so big.”

His loving Mother Tammy Travis,
His siblings: Zachary Matthews, Heather Matthews, and Brittany Carminati
Paternal grandmother Susan Travis
Maternal Grandparents David and Kathleen Mann

Also many loving aunts, uncles, and cousins.

Derek is predeceased by his sister Jackie Mann and his aunt Cathy Matthews.

Visitation will be held at the Gray-Parker Funeral Home, Inc. 100 E. Main St., Port Jervis, NY 12771 on Monday, January 25, 2021 from 2:00pm – 6:00pm.

Funeral services will be held at the funeral home on Monday, January 25, 2021 at 6:00pm at the funeral home.

Social distancing guidelines will be in place, a certain number of people will be allowed in the funeral home at one time, facemasks are required.

Cremation will be held at MacLennan Hall Crematorium, Milford, PA.
