There are two documents about Mr. Murphy's death. One is a Coroner and Death records log; the other is an individual Standard Certificate of Death. Both have the same information and are available from the State of Arizona History and Archives Division. The ones filling out the document left parts unanswered; however, the place of death was Phoenix City, Maricopa County, Arizona State. The name listed was Jerry Murphy and the address of Sister's Hospital. Identified as male, and color was American. The burial place was a Catholic Cemetery. The undertaker was Easterling and Whitney. The date of Death was April 21, 1905, and Tuberculosis was the cause of death. Dr. H.A. Hughes was the attendee. The parts missing are his age, birthplace, marital status, and the duration of the illness. The filling date and the Registrar are left blank.
