JERRY G. KOLLARIK
1938 - 2007

Jerry G. Kollarik, formerly of Toledo, Ohio and Pensacola, died Wednesday, August 22, 2007 at his daughter's home in North Huntingdon, PA.
He was born August 16, 1938 in Toledo, Ohio, a son of the late Stephan and Colice Bray Kollarik. Prior to his retirement in 1993 Jerry was employed by Toms Tum Associates and a 37 year veteran of the U.S. Navy which he joined on October 19, 1955. In 1970, Jerry enlisted in the reserved followed by active duty service in 1977, where he later retired on October 1, 1993. While in the Navy Jerry was stationed on the USS Forrestal, CVA 59 for many years. He was a Vietnam Veteran, a member of the Forrestal Reunion Association and the Non Commissioned Officers Association
In addition to his parents, he was preceded in death by his wife, Evelyn Lewis Kollarik in July 2006.
He is survived by a son, Jerry and his wife Nichole of Pensacola; a daughter, Rose McKinley and her husband Kevin of North Huntingdon; three grandchildren, Julie and Joshua McKinley, Aaron M. Kollarik; three brothers, George of Lakehurst, N.J., Gilbert of Toledo, Ohio and Leuis Kollarik of Norfolk, VA; five sisters, Nancy Trenchick of Mateca, CA, Mary McLaughlin of Northwood, Ohio, Janice Kollarik of Toledo, Ohio, Paulla Gladiux of Oregon, Ohio, Phyllis Swartz of Toledo, Ohio.
Friends will be received at the James W. Shirley Funeral Home, North Huntingdon, PA from 6 to 8 p.m. Thursday.
Interment will take place 10 a.m. Friday at National Cemetery of the Alleghenies, Bridgeville, PA.
Memorials to the American Cancer Society.
