Carol Ann Koslofsky age 74 of Thedford, NE passed away Tuesday, March 17, 2020 at the Great Plains Health Hospital in North Platte, NE.
Carol was born February 3, 1946 in Dunning, NE to Floyd Leland and Esther Louise (Haddix) Herbaugh. Carol graduated from Thedford High School and then went on to get her associates degree in Electronics.
August 21, 1986 she was united in marriage to John Hayes, to this union two daughters were born Jade and Esther.
Carol is survived by her daughters Jade Howard of Omaha and Esther Hayes of North Platte, one brother Lewis Herbaugh, two sisters Frances Martin, Fran Shaul.
Carol was preceded in death by her husband John, parents.
