The Hebrew date of death has two obvious spelling mistakes, both substituting the Hebrew letter Mem for the similar-looking letter Tet. It's trying to say Niftar 18 Sh'vat [5]757 (died 26 Jan 1997 or after sunset 25 Jan 1997). What it actually says is Nifmar 18 Sh'vam [5]757 (let's talk 18 [nonexistent month] [5]757).
