Heather Dianne Hammer, 43, of Loves Park, Illinois, passed away Tuesday, June 12, 2018 at her residence with family by her side. Heather was born on May 25, 1975 in Dixon, Illinois, the daughter of Jerry L. and Dianne J. (Levy) Hammer.
Heather worked as a caretaker and loved helping people. She was very artistic, loved to draw, paint and do crafts. Heather loved to go to the casino's and fish, but her greatest joy was seeing her children achieve their accomplishments in life.
Heather is survived by: her daughter, Haley Hall; son, Trevor Hall; parents, Jerry and Dianne; brothers, James K. (Angie) Hammer and Jeffrey S. Hammer; sister-in-law, Natalie Hammer; 4 nieces and 2 nephews: very special friend, Rose Martin.
Heather was preceded in death by: her brother, J. Michael Hammer; ex-husband Michael "Tony" Hall.
Service to celebrate Heather's life on Monday, June 18, 2018 at Sunset Funeral Home. Burial in Sunset Memorial Gardens.
