Tammy is survived by her sons Sean Pournik and Michael Graham; daughters Jennifer Graham and Brittany Graham; former husband Jeff Grady; grandchildren Jayden, Deonte, Raeleigh, Righlyn, Maddy, Jeremiah, Jacobe, Lexi, Jaecion, Ne’Kai, Brayson; sisters Diane Moore and Linda. Funeral services are scheduled for Tuesday, August 15, 2017 at 1:00p.m. in Greiner Funeral Home, 2005 North 13th Street, Terre Haute, Indiana, 47804. Burial will follow at Grandview Cemetery. Visitation will be at the funeral home on Tuesday, from 11:00a.m. until service time.
