At the "Yuin Reef" Royal Standard Gold Mine, Roy "Jack" was gaining petrol from a drum in the engine room. There was a lit miner's carbine lamp nearby which exnighted the petrol fumes, causing an explosion. Roy was covered in petrol, causing severe burns to his body.

Roy was transported to Mullewa via car, which was held up by a flooded river. On arrival in Mullewa, he was taken to the District Hospital, where he died later that evening. Roy's body was transported by train back to his native Albany, where he was buried in the family plot.
