Toby Jonathon 'T.J.' Smith, 38, of Willacoochee, passed away Thursday, April 13, 2017, at Southeast Georgia Health System in Brunswick.
He is survived by his mother Carol Smith of Willacochee, 2 daughters, Madison Gardner and Ansley Smith, his aunts and uncles, Barbara Woods, Toby Gale (Annie), Janis Tyler, Deborah Garner (Steve), 2 grandchildren, and several cousins. T.J. was preceded in death by his father, Larry Waymon Smith.
Funeral services will be held Monday, April 17, 2017, 2:00 pm, at Chapman Funeral Chapel, with Rev. Toby Gale officiating. A visitation for family and friends will be held one hour prior to service time. Private burial will be held at Palmetto Cemetery.
In lieu of flowers, the family has asked that memorial contributions be made to Chapman Funeral Chapel, to help with funeral expenses.
