A theory RE this mystery wife "Faye."
His wife's sister was named Winifred Faye Graham. Winnie was a widow when Theodore died, and she died shortly after Theodore. Perhaps she was the informant on his death information and whoever translated the Death Index made a typo?
Just a thought I thought I'd share.
Thanks for keeping up these profiles! :)
