He was born Saturday, May 11, 1940 in Grant county, and married Nancy E. Welsh Sunday June 19, 1960 in Van Buren.
Mr. Cook was a 1958 graduate of Van Buren High School, and retired from Ameritech in 1994 after 25 years.
He was a member of the Van Buren Methodist church and held many different positions at the church.
Mr. Cook enjoyed being outdoors camping, fishing and hunting. He was a member of the Mississinewa 1812 Battlefield Society and liked watching NASCAR.
Survivors include his wife, Van Buren, son, Matthew (Colleen), Fishers; daughters Deborah (Keith) Spaulding, Fairmount, and Kelly Ann Herring, Fort Wayne; seven grandchildren; two great grandchildren; and brothers Phillip (Connie), Lady Lake, FL., and Byron (Judy), Marion.
He was preceded in death by a son, John William.
Services were Monday, Jan. 16 at Van Buren Methodist church with pastor Cindy Osgood officiating.
Interment was in Marion.
