Four airmen lost their lives when 1 CNS's Anson (#6069) aircraft hit the ground near Newdale, Manitoba, Canada. The aircrew of the Anson were on a navigation exercise when the accident happened.

Sergeant Adam John Mazur is commemorated on Page 95 of Canada's Second World War Book of Remembrance.

A special cairn displaying a plaque, which commemorates the four airmen who died on service, was erected in Cardale, not far from the location where the Anson crashed.
