Born in Peekskill, New York on July, 23, 1940, he spent most of his life in northern New Jersey. Jack went on to graduate from Fairleigh Dickinson U. with a Bachelors of Education and Paterson State U. with a Masters in Reading Ed. He was an educator in NJ before moving to VA in 1984, as a National Sales Manager for several years, he later returned to teaching where he retired from Hampton Public Schools. A member of the Church of Jesus Christ of Latter Day Saints, he served as Bishop, Regional Humanitarian Representative, and various other positions within the church. In 2003 he founded The Needs Network for charitable and humanitarian efforts. Survivors include his wife of 54 years, June Chase; children; Dawn Marie Carmichael, Tracey Lynn Lazarus, Kimberly Ann Hornbaker, and Mark Leverne Chase; siblings: Barbara Hadden, Cathy Negri, David Chase, and Wendy Chase; 16 grandchildren; and 12 great-grandchildren.
