Manner of death was edema of the brain, following an accident, specifically, automobile vs. pedestrian. Source: INDIANA STATE BOARD OF HEALTH CERTIFICATE OF DEATH #2986.
His middle initial is a question mark to me. On the death certificate it looks like an "F", however other sources say it is a "T". If anyone can clear this up, please contact me.
The death certificate does not list a specific cemetery for his burial location. It simply says, Valparaiso, Indiana. He may be buried in the Adams Cemetery, however, I did not find any proof of that. If anyone knows his burial location, please contact me and I will make the correction.
