Booneville, KY- Blake C. McIntosh, age 79, the son of Oscar and Candus McIntosh, was born on January 24, 1937 in Lerose, KY. He entered into eternal rest on September 11, 2016.
Blake McIntosh is survived by his children: Amy (and Matt) with children David and Natalie of Indiana, June, Susie, Blake Jr., and Sidney in Florida, along with many grandchildren and great-grandchildren. Blake is survived by one brother, Robert G. McIntosh of Ohio. Blake is also survived by Maxine Smith of Indiana, a loving caregiver and spouse of three years.
Visitation will be held Wednesday, September 14, 2016 from 5:00 to 8:00 pm at the Booneville Funeral Home. Funeral services will be held Thursday, September 15, 2016 at 11:00 am at the Booneville Funeral Home. He will be laid to rest in the Pleasant Grove Cemetery located in Booneville, KY.
