Born to Argent Butler (1839-1900) and Matilda Mantle (1838-1927). The family was living in Payson when Ada was born. By 1910 she was married to James Forbes and they were living in Salt Lake City. In 1920 they were in Bingham and then back to Salt Lake in 1930 before James died in 1932.

Ada died of heart failure in 1961 at the age of 84. Ada was cremated and her cremains were buried on top of James' grave.
