She is survived by her husband of 20 years, Doyle (Tad) Polk, mother Martha Hess (Sonny), brother-in-law Chris Cherry, niece Bethany Wills (Billy), nephew Christopher, step-son Travis Polk (Chelsea) and grandson Finley. Donna also leaves behind a host of relatives in her beloved North Carolina.
She was predeceased by her father, Donald Clark, and sister, Teresa Cherry.
Donna was first and foremost a teacher. Her many years at Chimney Lakes Elementary touched the lives of countless children. She loved her church, Lake Shore Baptist, and was supported throughout her illness by her church family, friends, family, and colleagues.
A Memorial Service was held at 2:00 pm on Thursday, June 23, 2016 in Lake Shore Baptist Church, 2365 Blanding Blvd, Jacksonville, FL 32210.
A graveside service will be held at 2:00 PM Saturday, June 25, 2016 at Dry Pond Cemetery in Marshall, NC.
