Hazel Monsky Wolfson, 95, passed away peacefully at River Garden, January 2, 2016 with her favorite song, "Always", playing softly. Born in Montgomery, Alabama to Abe and Jennie Brockman Monsky.

Hazel was a long-time resident of Jacksonville, Florida, having moved to Jacksonville in 1940 upon her marriage to the dashing Saul Wolfson.

Affectionately known as Hazy, by her late husband, Saul; Hazel was also predeceased by son, Richard (Lesley) Wolfson; and by her siblings, Florence Monsky Wolfson, David Monsky and Doran Weinstein. She is survived by children, Maura Wolfson-Foster, and Donald (Karen) Wolfson; grandchildren, Jordan Foster, Scott and Jeffrey Wolfson, Felicia Sias (Michael), and Kymberly (Kal Kseib), Erin, and Lance Wolfson; six great-grandchildren; one great-great grandson; cousins, John (Joan) Monsky and Ralph (Bronia) Lowenstein; and many loved nieces and nephews.

A generous and active civic volunteer, Hazel was known for her Southern charm and beauty. She was a founding member of The Women's Board of Wolfson Children's Hospital, a member of the Wolfson Children's Hospital Auxiliary, American Cancer Society, Children's Home Society, Jacksonville Art Museum, Temple Sisterhood, Council of Jewish Women, Hadassah, and a member of Congregation Ahavath Chesed.

The service will be held at The Oaklawn Chapel, 4801 San Jose Blvd., Jacksonville, Florida on Tuesday, January 5, 2016 at 2:30pm, with entombment to follow in Oaklawn Cemetery. In lieu of flowers, memorial gifts may be made through The Women's Board for the Saul and Hazel Wolfson Endowment for Pediatric Research at Wolfson Children's Hospital, 1325 San Marco Blvd., Jacksonville, FL 32207; or to River Garden, 11401 Old Saint Augustine Rd, Jacksonville, FL 32258.

The family expresses deep appreciation to all who loved and cared for Hazel, especially Marty Goetz, and those who provided her loving care at River Garden.
