There is a blurb in “The Violinist”, volume 6-7, p. 41 from 1908 that reads:

Waukegan, Illinois

The “Mandy Orchestra Club” will be heard in concert April 20, with Mr. Urban Burridge as soloist. On Tuesday evening, March 23, Mr. Frank P. Mandy’s pupils will be heard in Recital.

In “A History of Lake County, Illinois” (1912), p. 804:
Mr. Frank P. Mandy, in charge of the Violin Department, is a Hungarian, born in that country, beginning the study of the violin at an early age, and studying under the best teachers. As a virtuoso and teacher Mr. Mandy has won an enviable reputation, standing among the best concert violinists of today. Urban Burridge, his assistant, is a graduate of the Waukegan Conservatory of Music.

In June, 1917 for the WW I draft registration card, he is listed as an “Inmate of Insane Hospital”. In the 1920 Census, he is listed as an Inmate at the Elgin State Hospital and it appears he passed away there in 1926.
