Nicholas George Cozakos, 66, former assistant director of the Utah Department of Health Speech and Hearing Clinic, died Tuesday, Oct. 30, 1990, at his Salt Lake home after a long illness.

He worked for the state agency for 35 years. Before retiring in 1987, he tested the speech, hearing and language function of thousands of Utah children.

Mr. Cozakos was legislative counselor for the American Speech Language-Hearing Association and served as an officer in the Utah Speech Language-Hearing Association, which honored him with a Distinguished Service Award in 1987. He was similarly honored by the Utah Public Health Association the same year.

He was active in musical groups, serving as director for 13 years of the Holy Trinity Greek Orthodox Church Choir. He founded the Greek Orthodox Church Choir Federation of the Western States, had performed with the Utah Symphony Chorus for the past 22 years and was an ardent supporter of musical and theatrical organizations.

The funeral will be Saturday, Nov. 3, at 2 p.m. in the Holy Trinity Greek Orthodox Cathedral, 279 S. 300 West. A prayer service is scheduled at 7:30 p.m. Friday in Evans & Early Mortuary, 574 E. 100 South, where friends may call from 6 to 8 p.m. Burial will be in Mt. Olivet Cemetery.

Friends may contribute to the Holy Trinity Cathedral Pipe Organ Fund, 2474 E. 900 South Circle, Salt Lake City, UT 84108, in memory of Mr. Cozakos.
