Theodore Edward Tuomi II, infant son of Mr. and Mrs. Theodore E. Tuomi, 1314 Revell Ave., died at 7:30 a.m. Monday, Oct. 22, 1962, in St. Anthony Hospital shortly after he was born.

Survivors include: his parents; three sisters, Cheryl Ann, 15; Ruth Alice, 7, and Suanne Carol, 5, at home; maternal grandparents, Mr. and Mrs. John Barber, Rockford; and paternal grandfather, Edward Tuomi, Rockford.

Graveside services at 2 p.m. Wednesday in Sunset Memorial Gardens.
