Thomas died on Jan. 29, 1914 after falling from a bridge. He was 63 years old, widowed, a Foreman of Construction, and lived at 120 S. Jones St. Thomas was buried at St. Mary’s Cemetery in section 4.21.1, next to his wife. The informant on his death certificate was Minnie E. O’Brien.

Fell From Bridge.
Found Unconscious and Drawn to Top of Pier With Rope in and Unconscious Condition.
Groans coming from the foot of the first pier of the river bridge attracted the attention of a night patrolman while making his rounds Thursday morning at 1 o’clock and upon investigation it was found that Thomas O’Brien of South Jones street had fallen through one of the man holes in the side of the bridge over the pier and landed on the ice 20 feet below. A sleighing party happened along about the time and when a rope was produced the driver slid down and fastened the rope about the body of the unconscious man and he was drawn to the bridge.
He was then taken to the police office and physicians sent for. He was later removed to his home and was still in an unconscious condition before noon.
Mr. O’Brien died since the above was put in type.
