Blaylock of Phoenix, AZ, grandchildren; Emerald Watts, Jennifer Chandler, Melissa Chandler, Aaron Chandler, Ashley Chandler, Alexis Chandler, Jeremy Blaylock, Jr., Joshua Blaylock, a great granddaughter Alexia Grace Rodriguez, two brothers, Leon and Eugene Tate and a sister Erma Lee Cothren.
