WAPAKONETA — Carol Kay Koch, 73, of Wapakoneta died at 10:13 p.m. Monday October 6, 2014 at her residence following an extended illness. She was born on July 27, 1941 to Ralph & Iva Perkins in Lyons, IN who preceded her in death. On August 16, 1964 she married Dean A. Koch and he passed away in 1986.

She is survived by a daughter Nancy (David Glen) Smith of Wapakoneta, a son Jeff (Christy) Smith of TN, a step-son H. Lee Koch of AK, a step-daughter Lois Ann Snider of Dayton, 8 grandchildren: James (Julie) Wortman, Rob (Tiff) Wortman, Melinda King, Ail (Nicole) Koch, Johny (Tina) Koch, Sarah, Ryan, Amy Smith, multiple great-grandchildren and 2 sisters, Norma Decker and Merle (Wendell) Hennesse, both of IN.

She was preceded in death by a brother Charles Perkins, 5 sisters: Thelma Strabley, Lenore Elsass, Melba Dobbs, Mary Perkins, Eva Laureman and a great-granddaughter Aleya King.

Carol retired from Ametek Westchester Plastics. She enjoyed spoiling her grandchildren and great-grandchildren.
