At the beginning of May the Battalion was participating in Operation PURPLE MARTIN in northern Quang Tri Province. On May 2, during the movement of Company F troops a mid-air collision occurred between a CH 46D from HMM-262 and a UH-1H, with the CH 46D crashing west of FSB Russell in the vicinity of Dong Tien Mountain and the damaged UH-1H managing to land. There were no survivors, in the CH 46 D, twelve passengers and crew died, one of the casualties was 1stSgt Sumner.
On May 19, members of Company B, 4th Marines recovered remains and personal items at a crash site which led to the identification of 1stSgt Sumner.

Viet 'Copter Crash Kills Former Local Resident
A veteran Marine, who was a former Jeffersonville resident and whose two brothers live here, was killed in a helicopter crash in South Vietnam, according to word received from the Marine commandant.
Mairen 1st Sgt. James H. Sumner, who served 23 years in the corps and who was on his third tour of combat duty; was killed May 2 when the helicopter in which he was a passenger collided in Mid air with another American 'copter.
The crash occurred 22 miles west of Don Hain Quang Tri Province, according to Gen. Leonard F. Chapman, Jr. On May 19,1969 members of Company B, 4th Marines recovered remains and personal items at a crash site which led to the identification of 1stSgt Sumner.
Sumner resided in Jeffersonville from 1942-46 and attended Jeffersonville High School for 3 years before moving to Scottsburg where he completed his high school studies.
The 41 year old Marine is survived by his wife, Carol Caudill Sumner (Married November 12, 1949, Clark County, IN) who has been living in Oceanside, Calif., during her husband's tour of duty in Vietnam; a daughter, Karey; and three brothers, A.R. John Sumner, and Jerry Sumner both of Jeffersonville, and Frank Sumner whose address is unknown.
Eternal Hills Memorial Cemetery, San Diego, California has been reported as burial location.
