Hazel was born on May 15, 1912 in Madison, the daughter of Ralph and Helena Hankey. Hazel married Alvin Jacobs on April 25, 1942 in Jefferson, Missouri. Alvin preceded her in death on April 27, 1974. Hazel was a member of St. John's Lutheran Church of Madison and a former member of the Eagles Club. Hazel also worked for many years until her retirement at the Madison Dairy Produce Company. Hazel passed away peacefully on May 4, 2014 while at Karmenta Nursing Home. Hazel is survived by a sister and two nephews. Hazel was preceded in death by her parents, husband, Alvin and two siblings, Olive Siewert and Ernest Hankey. Cress Cremation Services.
