Mr. Formyduval attended Palmyra Baptist Church, Old Dock, N.C. and served in the Army during WW II.
Survivors include two sons, Ishamael Formyduval, Jr. and wife, Phyllis of South Port, N.C. and Kim Formyduval and wife, Alice of Columbia, S.C.; Daughter: Jewel Formyduval Dart and husband, John of Columbia, S.C.; Brother: Samuel Formyduval and wife, Bettie of Shallotte, N.C.; Sisters: Geraldine Inman and husband, Larry of Charlotte, N.C. and Patricia Anderson and husband, Jimmy of Raleigh, N.C.; Nephew: Adam Formyduval of Shallotte, N.C.; Six Grandchildren and two Great-Grandchildren and many nieces and nephews.
Funeral Service will be held Saturday, April 5, 2014 at 2:00 p.m. at McKenzie Mortuary Chapel with Rev. Ken Sides officiating. Burial will be in the Columbus Memorial Park Cemetery.
