Hazel Mullins
Hazel Evelyn (Cook) Mullins, 48, Kenvir, died April 19, 1999 at the University of Kentucky Medical Center in Lexington.
A native of Evarts, she was a homemaker and believed in the Pentecostal faith.
She was a graduate of Evarts High School.
She was preceded in death by her parents, Robert Cook and Hailey Marie (Patrick) Cook; and a brother, Carl Cook.
Survivors include her husband, Melvin Mullins, Kenvir; two children: Troy Dewayne Mullins and Molly Katrina Mullins, both of Kenvir; father-in-law, Fred Mullins, Kenvir; mother-in-law, Thelma Mullins, Kenvir; and four sisters: Helen Wynn, Harlan; Shirley Napier, Lousiville; Geraldine Cook, Chicago, Il.; and Bobby Cook, Stone Park, Il.
Visitation is today from 5 to 9 p.m. at Treblo's Pentecostal Church at Evarts.
The funeral is Thursday at noon at the church with the reverends George Pace and James "Jabo" Madden officiating.
Burial will follow in Evarts Memorial Gardens.
Pallbearers are Mark Pace, David Lockhart, John Cox, Harry Southard, Danny Southard, Mark Napier, Mark Lewis and Mark Ray.
Evarts Funeral Home is in charge of arrangements.
