Some sources suggest Frank was born in New Hampshire; others state he was born in Vermont. His parents were living in Weathersfield, Vermont by 1838, which was about 2 years before he was born.
Frank enlisted on August 22, 1861. On September 21, he mustered in as a Private in Company C, 4th Regiment, Vermont Volunteers. He was reported missing in action at Lee's Mill (Virginia) on April 16, 1862, and died from the leg wounds he received around that same day.
The memorial in Ascutneyville Cemetery is a cenotaph. Frank is interred at Hampton National Cemetery.
