John enlisted in the 15th Independent Battery, Ohio Volunteer Light Artilley, December 4, 1861, for a term of 3 years. Sustained a gunshot wound to the middle of his right femur, April 19, 1863, at the Battle of Coldwater, Mississippi. After treatment at other hospitals, he was admitted to the U.S. General Hospital at Cleveland, Ohio, Nov. 5, 1864. The Casualty Sheet in his service record from the National Archives shows that he died in the U.S. General Hospital at Cleveland, Ohio, January 14, 1865. Cause of death: "Suicide committed by cutting his throat with a razor." Note that his name is spelled "Maddox" in the Official Roster of Ohio Soldiers but is spelled "Maddox," "Mattock," and "Mattocks" in army records. Other gravestones in Garfield cemetery of obvious family members show "Mattucks."
