The death certificate of Thomas Jefferson Sherlock states he died from Chronic Interstitial Nephritis @ age 70y 2m 1d of age. He was buried in the Hart Cemetery. It also states he was a physician, born on Sept. 18, 1845 in Penn., and died Nov. 19, 1915 at Butterworth Hospital in Grand Rapids, Kent Co., Michigan.
