MADISON --Mrs. Hazel Cosby, 64, died Thursday at Methodist Evangelical Hospital in Louisville.

Survivors include her husband, James E. Cosby, Sr.; six sons, James E. Cosby, Jr. of Wabash, Kenneth, Steven, Thomas, Jeffery and Marvin Cosby; three daughters, Bonita Malone of Hanover, Vanessa Tyree of Louisville and Faye Cosby; three brothers, Wilson Watson of California, Leon and Wilbur Cosby, both of Hanover; four sisters, Mrs. Helen Chandler, Katherine and Virginia Watson, and Janell Duncan; and 25 grandchildren.

The funeral will be at 2:30 p.m. Sunday at Ebenezer Methodist Church, with burial in Springdale Cemetery.

Visitation at Lytle Mortuary will be from 2 to 9 p.m. Saturday.
