Jacob Everrett Andrew West, 17, La Porte passed away at 6:38 pm Wednesday, September 25, 2013 at IU Health La Porte Hospital.

He was born July 11, 1996 in Indianapolis to Scott and Julie (Bales) West and lived most of his life in the La Porte area.

He was a junior at La Porte High School and played varsity football and was a varsity lacrosse player. He was a member of The Fellowship of Christian Athletes.

Jacob was a member of Bethany Lutheran Church.

He leaves behind his mother, Julie (Brett) Schroeder, La Porte; father, Scott (Allison) West, Indianapolis; two sisters, Courtney and Kate West; and one brother, Tommy West; maternal grandmother, Phyllis Bales; paternal grandparents, Bob and Carole Schroeder, several aunts, uncles and cousins.

He was preceded in death by his maternal grandfather, Robert "Gale" Bales, and paternal grandparents, Dick and Wanda West.

A celebration of life service will be held at 4:30 p.m. Sunday, September 29, 2013 at Kiwanis Field, La Porte, Ind. with Pastor Dennis Meyer officiating.

Private family entombment will take place at Swan Lake Memorial Gardens, Michigan City, Ind.

Essling Funeral Home is handling the arrangements.
