Aviation Machinist's Mate Petty Officer 1st Class Affield was one of seven killed when their Lockheed P-3B-65-LO Orion crashed 20 miles NE of Terceira-Lajes AFB, Azores during a routine training mission.

He is represented on the Remembrance Rug by symbol #6310.
