Tammy Jo Shaw, of Faulkner Rd., RD 1, Olean, died Sunday (Dec. 28, 1975) at the Olean General Hospital following a brief illness.
She was born at Bethesda, Md., Feb. 3, 1963, a daughter of Theodore and Diane Faulkner Shaw.
She was a student in the seventh grade at Portville Central School and attended Kingdom Hall, Jehovah's Witnesses at Olean.
Surviving are her parents, four brothers, Theodore Shaw, Terry Shaw, Todd Shaw and Thomas Shaw, all at home; maternal grandmother, Mrs. Evelyn Faulkner of Olean; paternal grandfather, Keith Shaw of Ceres; and paternal great-grandmother, Mrs. Maude Shaw of Ceres.
Friends may call at the Guenther Funeral Home Inc., Portville, Tuesday from 2 to 4 and 7 to 9 p.m. Funeral and committal services will be held there Wednesday (Dec. 31, 1975) at 1:30 p.m. The Rev. James H. Miller, will officiate. Burial will be in Chestnut Hill Cemetery, Portville.
