Find A Grave Memorial# 122346734 is a cenotaph and I have submitted a request to have it marked as such.
Therefore, Find A Grave Memorial# 145614642 is NOT a duplicate. It is a memorial page for where his remains were interred.
On FindAGrave it is acceptable to have more than one memorial page when markers or memorial stones have been placed for an individual in more than one cemetery:
There are some situations where an individual (usually a 'famous' individual) may have more than one memorial page, because their body was buried in one location but a cenotaph or second marker/memorial was also placed in a second location.

The death certificate showing mother's maiden name is listed as WEST is incorrect. Her maiden name was Williams.

My apologies, I should have been more clear. You posted a message "A note on William Crawford Blalock #90702270" about Crawford's death certificate in which son Robert names his own mother Nancy, instead of Crawford's mother Ella. You said they were your grandparents, but perhaps you were relaying a message from someone else. I have seen this error many times on death certificates.

Crawford and Nancy were my grandparents. How could they be yours? None of their daughters married a Thompson.
